I don't think it's necessarily a fistula (or the the fistula you had, you know I didn't realize this but I found out that once you have a fistula it stays forever and that it can go into remission because when I asked my GI about my fistula from the last MRI I had, I asked if the fistula was gone and he said, yes the inflammation from it is gone BUT the tract stays where it is indefinitely, that freaked me out honestly cuz I assumed that the actually completely disappeared or healed and the same area wouldn't necessarily be affected by it ever again...weird eh!)
anyway, as I was going to say, your story sounds close to mine (I too am on Humira but only Humira) so I still "go" more than you do in a day and I'm constipated (since I developed my fistula almost a couple of yrs ago now) and my GI reminded me that 14 yrs ago (I've been sick for 24 yrs now) he suspected I had developed IBS (at that point we didn't know if it was alternating IBS, IBS-constipation or IBS-diarrhea at that time because as usual I was flaring with my CD but my GI detected something else was actually going on besides the crohn's (I have crohn's colits, my CD is affecting my colon...on and off in the rectum for 23 yrs straight until Humira put me in remission) but now that my CD is in remission we've discovered I have IBS-alternating but it's constipation more so than diarrhea. And your symptoms you've described are very similar to how I've been physically feeling.
When was your last scope? are you in remission with your CD?
If you are then you could very likely be exhibiting IBS, it's not all that uncommon for IBD and IBS to coexist....trust me....nothing worse than finding out your in remission and still feeling gross with urgency and frequency and it always being constipated, I'm always scared cuz I sometimes get weird vaginal feelings like I did when the fistula was active and it scares the crap out of me....literally!
