Doing a search about legal issue

to see if there is any information on the forum about the basics in what to include in a letter to the court about my Igenex lyme diagnoses. I have to show I have lyme and talk about what it can do what it can lead to and what it has done to my health. I dont want to go into much detail about Igenex diagnoses because of the political stuff surrounding late stage lyme and the CDC's backward wrong headed thinking.
This is not for disability but for another legal matter that if I do not prevail I will not be financially able to continue my supplements and herbal tinctures. I am on the Bhuner protocol
My healthcare practitioner has been out of the state traveling and does not have time to do it but says if I write a draft she will edit and amend it on her stationary
If anyone has any suggestions I would appreciate any help or advise. thanks junie
