I hope you can get through the herxing. You shouldn't be ratcheting down like that for too long. Like I may have mentioned, you probably need to kick your detoxing up a few notches. I'd do a post asking for advice—people on this forum have shared quite a few clever detoxing protocols.
And I hate that you have GI problems too. It is a terrible club to belong to—when you have to eat 3 times a day, it is the most disruptive of all my sx. I don't think I was not tested for yersinia but one of my docs did suspect it along with so many other conditions. I hope you aren't having too much interfere with the Rocephin. I have had no trouble with the infusions so far although yesterday I did have nausea for quite a few hours.
How log will you be getting the IV infusions? I understand the PICC is fairly easy to wrap up with saran wrap for showering although maybe a little fussier for day-to-day. I have horrible veins that are deep and close up so drawing blood (which I have to get done ever 2 wks or so) is a huge hassle. This has contributed to my fear and hatred of needles—doesn't help that the phlebotomist has to move that #%#* thing around once she's stuck me for the 4th time, still can't get any blood so we have to try again. I generally have blood taken either from my hand or wrist when the arms don't work. It's all just too much.

youtu.be/FqYJC1J6qksAnd this video shows how the port is "accessed" and "de-access" once a week (this patient doesn't have the huber needle in place all the time like I do) but the video shows how the needle is changed out once a week, how the port is flushed out with saline and heparin (which I do daily before/after I inject the abx) and also shows how blood can be draw for labs.
