it sucks going thru the herxing but it is usually a good indication the medicine is working. It's all about
finding the right middle ground where you herx but not to the point where you feel you are getting hit by a freight train.
These treatments can be marathons, so I wouldn't hesitate to start the meds slowly and gradually build up, sometimes it takes months to build up. It is a pretty consistent strategy that has allowed people to treat their otherwise intractable diseases with meds that hit the disease square in the jaw, causing overwhelming herxes, but getting hit that hard can dissuade people from continuing treatment.
Hopefully your doctor can help you with that strategy, but I would try and stay on the meds if possible. I'm not a doctor, but that's just my lay person opinion and based on my own experiences (which obviously cannot be universally applied haha)! Another strategy I suppose, is your dr might have other gentler meds to help whittle down the pathogen, and then down the line you could try reintroducing the malarone, and hopefully with a reduced pathogen load the herxes won't be as strong.
Oh yeah, and like Trav says, try to detox as much as possible!!!
Good luck and keep hanging in there!
