I never had any problem. I was at work, the next day I called in sick (really thinking it would be one day), then called in to say I'd be out for a week, then went on Short-term Disability (at this point I was seeing a LL neurologist who did ALL the paperwork for me), then Long-term Disability. As I said, my new doctor did all the paperwork and I had no problems on that side of the equation. Perhaps it varies by state?
PS I just read the linked document in the post prior to mine. Since I had disability through my employer, it was 'that' insurance company's paperwork that the doctor first filled out and they complied albeit after many months they would call to try to 'close the case with the option to re
open' and send a lump sum of money; of course I kindly refused saying I'd be going back to work soon. Long story short, when I turned 65 (66?) they forced me to go on Social Security Disability, which was a fraction of the money I had received (67% of current salary when disabled), and THEY did all the paperwork to make it happen.
In truth I always thought I'd be going back; never knew what hit me nor it's long term characteristics. That was back in 2008; now, you can find excellent videos on YouTube, such as the presentation to Duchess County by the Harvard (Columbia?) University Hospital medical director....
