You should definitely get the viral loads down before starting Lyme & co treatment - they can easily overburden your immune system and render treatment ineffective. For me, fatigue is generally due to the viral load so managing this helps reduce some sx.
To do this quickly, use antiviral Rx. You should see progress within a week or so - if not try higher doses or a different antiviral (took me awhile to get what I needed).
There are also powerful herbs that are easier on the body that you can transition to once you make some progress. You'll want to have your liver enzymes tested monthly - and take milk thistle seed and/or burdock root to help it metabolize the antiviral.
And with every antimicrobials component you take, you need to implement a certain level of detoxing - to match it, in a way. Always start slowly and steadily increase.
Your MTHFR hetero mutation is specefic, but most MDs check for only two - while there can be hundreds of mutations that affect how we naturally detox and process methylation. So, IMO the test your MD ran isn't useful.
Hetero mutations are common and usually suggest about
a 30% impairment of the function. Most lymees have detox and methylation problems so that's why we focus on it as much as the antimicrobial treatment. If you were homozygous, this would suggest 60% impairment and might warrant further investigation.
Vit D levels are a bit controversial. Lyme likes D so you might feel better supplementing, others don't.
