Rifampacin , Azithromycin and doxycyline.

Rifampacin , Azithromycin and doxycyline.

This is my new protocol for Lyme and coinfections.

Just wanted to check if this combination is ok?

You might want to include your dose sizes for each as well, so that others can help you double check that as well. I don't know enough about the abx to be of any help, sorry, but this will bump your post up so that others are more likely to see it.

ok thanks

Azithromycin 600 mg monday wednesday and friday
Doxycycline 200mg once a day every day
Rifampacin 300mg twice a day

Any help much appreciated

It's a solid protocol. It should hit Borrelia, both as spirochetes and persisters, since Rifampin seems to add an anti-persister mechanism to Doxcycline.

It will also hit other infectious bacteria such as Ehrlichia, Anaplasma, Bartonella, etc.

Also, the Doxycycline dose could eventually be doubled (200mg BID), but better to start low.

Make sure you're taking good probiotics, and try to avoid sugar.
Also stay out of the sun while on Doxy.

I am on Artimisia also for babesia. I was on malerone but the herx was crazy so im off it now!

david - pairing Rifampin with doxy - The Rifampin may decrease the effectiveness of doxy by decreasing the blood levels.

But, some LLMD's still do prescribe them together.

I would be more concerned if it was the other way around (if Rifampin was being decreased) - as you do have another antibiotic (azithromycin) to go with it.


FWIW - I have also read that Rifampin can decrease the levels of Biaxin in the blood, and Dr. M (Galaxy lab) uses those two together when treating bartonella.

I agree with david - increasing doxy would be a good idea - especially knowing about the decreased levels that may happen due to Rifampin.

I suggest you start one at a time, give it at least a week before adding second...etc.



And, the Rifampin - when I started it, I was instructed half dose to start...

You make a good point Girlie.

I'm finding the more I read blogs and hear about different doctor's protocols, the more I find doctors don't prescribe based on monograph interactions, but rather on what combinations work for them in the clinical practice.

Great advice guys thank you so much.

I will continue with Azithromycin and Doxycyline as been on them for couple months now but will slowly add in rifampicin starting half dose like you say. I will also slowly increase the doxycyline dose around 4-6 weeks into rifampicin.

I was thinking of adding grapeseed extract. Is this a good idea?

I believe grapeseed interacts with a number of medications via p450 inhibition/induction, can't remember.
Might want to google that one first.

Just want to clarify you meant Grapeseed extract and not Grapefruit seed extract for cyst busting?

MS - I think you must be referring to GSE.


Grapeseed extract is an antioxidant - not sure what other properties it has without looking it up.

I have read that Rifampin is a cyst buster - but not sure if that can be relied on - if it's enough - most LLMD's use Flagyl or Tinidazole.

Have you had a discussion with your LLMD about treating cyst form?

Cycle your artemesia - 1 week on, 1 week off, or 2 on 3 off. It loses effectiveness after 10 days and your system needs to reboot.

Watch your sun exposure with doxy. If you normally take 60 minutes to start a burn (unscreened) then you will likely burn in 20 or less. Ears especially!

After I took multiple cycles of doxy it started peripheral neuropathy. I'm now on Zith.

Watch for reactions.

Old Salty - that's Artemisinin that needs to be pulsed - not Artemisia - which is the whole herb.

Did your neuropathy go away once you discontinued the doxy?

I couldn't remember if grapeseed interacts with certain antibiotics, but I do know Grapefruit Seed definitely does interact.

I would avoid that while on antibiotics.

Girlie is right, Tindamax (Tinidazole) or Flagyl (Metronidazole) could be cycled in for one of the three antibiotics at a later date, but I'd personally give it a bit and see where you get.
