My Girlie is finally taking azithromycin. She tolerate it better than biaxin and bactrim, however her llmd has prescribed it with a strange dossage.

500mg 3 days in a row at start and then 1 day on and 1 off. He says that it's not pulsing, because it has 68 half life, and it's enough to mantain mic levels. Is it true?

I don't know if I agree with that. Azithromycin shouldn't be pulsed as far as I hear from LLMDs that are lead researches within the field (Lyme and Co-infections). Also, Azithromycin can slow the Q-T wave length, so I don't know if I would do it at such a high dose.

Additionally, what is he/she trying to treat your daughter for? Does she have any neurological symptoms? If so, Azithromycin doesn't get into the central nervous system, so it is usually used with another antibiotic. Most LLMDs in the know try to use two antibiotics that both get into the central nervous system if their patients are having neurological symptoms.

She's pairing it with rifampin, but it penetrate well brain tissue.

I'm not concern about qt, zithromax is the safer macrolides and new studys doesn't demostrare high mortality in patients taking azitro.

Eritro and clarithromycin are cyp inhibitors and can block potassium chanells and ocasionally cause long qt syndrome and sometimes torsade de points but azitro is not cleared by cyp and can prolong small qt by itself but not causing major concerns
