How long does it take to know a treatment is working?

Hi, started treatment for bartonella and borrelia. (20 mg DOX 2x daily , 250 mg azithromycin 1X) and am 1 week in. I do notice a reduction in the symptoms, but they are not gone.

I have a follow up in 1.5 months and am not sure If I should wait that long to provide feedback and ask the physician to evaluate.

most LLMDs say to treat with at least 2 or 3 antibiotic combinations for a minimum of 4 months if Lyme and Bart are suspected

many people find they have to treat for much longer

the fact that you have reduced symptoms already is a positive sign but it's very early days in the treatment to draw any conclusions.

I would just try not to overthink it in the meantime and do everything you can to support your body in the healing process.

ps i think you likely mean 200mg of doxy 2x

Thanks Garzie, you read correctly, my regimen of dox is 20mg.

So far, the frequency and severity of the shooting pains in the scalp, heel, and shins are reduced, but not gone. Not much improvement on my throat pain, circulation, or relief from the cold sweat and general feeling of being too cold.

That’s an odd dosage. Way too low

It should be 200 mg twice daily
(Some non LLMD’s prescribe 100 mg twice daily)

I think they set it low because of my genetic testing results predicting that I’d be somewhat intolerant of the dox. I can say so far this low dose hasn’t caused me problems. I’m only a few weeks in and I can tell I am better than before treatment, but I’m still having symptoms so the Lyme and Bartonella are probably still kicking around to some degree.

My concerns are - could the very low dose cause resistance.

Is there a plan to titrate up?

Maybe mention this to your Doc when you see him in a few weeks?
