Ermi test

I need a bit of help with what forms to check off and send back to Mycometrics for the dust test. My head is not very clear today! The forms I've printed out are:

Chain of custody form
(it asks for sample id?) do i fill out every room I need sampled. The forms are confusing to me. The chain of custody form aske for client info which I assume they need asap with the check I mail. But it also asks for "date sampled" which I wouldn't know until the kit gets here. Plus there is not enough room in the column to write all the places I will sample.

I would assume these stay with me.

I think the company just isn't very good at the website game and at making things clear. I'll try calling them and getting some answers. Thanks for listening.
