Hi. I’ve been on Azithromycin for a while, with rifampin, methylene blue, and tindamax. My Lyme doc suggested switching Azithromycin for clarithromycin. The first day on clarithromycin I had more twitching than usual, my anxiety went up, and my feet got hot (which I haven’t had happen in 6 months). My question is is this a herx or bartonella is moving around more? Maybe the Azithromycin was a better fit for bartonella, or was this a herx with Clarithromycin. I’m concerned. Has anyone had good success with Clarithromycin? Thank you kindly.

It sounds like a herx to me.

I have taken Clarithromycin - it was one of several abx I took and I am now in remission.

Thank you for your response. Did you you have bartonella symptoms when you were on clarithromycin? Twitching etc? And do you think an increase of twitching can be a herx? Thank you.

Yes, I had Bart symptoms on clarithromycin (I took it when I was treated by the first Lyme doc I saw) - before going to DrJ
Most definitely an increase of twitching can be a herx.

another vote for most likely herx reactions

some feel clarithromycin works better for some infections than for others
its in the same family of abx as azithro so works on the same bacteria by the same mechanisms - but has different pharma kinetics - ie half life and tissue penetration etc
so this could make it more effective for some bacteria
the standard dose is also higher than azithro which could have an impact
( standard doses are not a precise science - more of a best guess )

I’ve been on Clarithromycin 5 days now. I noticed a herx the first two days. Do you think I’ll see changes in twitching frequency over time? Like it lessening? Did you notice improvements with it weeks to months in? I guess 5 days is not too long. I just don’t feel like the twitching has lessened much. Thank you.
