When in doubt, I always test the CD57 because it's a cheap test for me, and even though it doesn't give me very clear answers, it's always interesting at least to know what's going on there.
Why would you consider doing the Igenex again? Was it negative the first time? Your doctor may not require a positive test in order to diagnose/treat Lyme - or do you just want to know for your own peace of mind?
I'd say, for the first visit, follow what your doctor suggests - each doctor has their own way of doing things - maybe you need to feel each other out a little bit, first? By the way - good luck tomorrow!
Other than a CBC which is always a good idea when starting off with a new doctor, I'd suggest a comprehensive digestive test (usually a take-home kit) as it can be really helpful to know what's going on in your gut while you're taking so many things orally to treat Lyme. Adrenal/hormonal testing can also be helpful, although my own advice is to be careful not to overtest yourself - sometimes I go a little crazy and just look for every abnormality I have and end up overwhelmed, trying to correct way too many things at once!
