The tilting back bothers my neck sometimes, too. Leaning forward over the kitchen sink is worse, as I get nerve pain that way.
Sometimes I have to just close my eyes and not tilt my head back...and let the water come straight down on my head. It runs down my face, too, so I put a cloth over my shut eyes...helps a bit.
I do a quick shampoo just at the top of my head - along the part line...that's usually where mine needs washing, the rest to the ends isn't oily. But, I am washing it every other day...so it's not getting to point where it's really bad.
If you do a quick wash at the very top, when you rinse, the shampoo runs down over the rest of the hair...it's not ideal...but it doesn't hurt as much (especially since you're not tilting back)...and the hair gets washed somewhat...and smells better.
