I agree with SpeyFisher on every point.
Like you, my symptoms became most apparent in my early years of teaching. I also attributed the fatigue to the job and the sheer stress of teaching responsibilities. Similar to your back pain, I had foot & leg pain for a few years but thought it was the heels I always wore to work or related the joint pain to my workouts. It was in these years that my heart issues acted up the most. Was also frequently prescribed amoxicillin or azithromycine (z-packs) for sinus infections and walking pneumonia, and would always feel *almost* better. Things quieted down for a few years, then weird symptoms of chronic "hangover" feeling or "morning sickness" feeling every 3 months or so for a few years. With my most recent and very stressful job, symptoms ramped up into high gear last summer, finally becoming debilitating. The heart palpitations were so strong they shook my couch or bed. Finally got the positive lyme diagnosis. The heart stuff cleared almost immediately on meds. Your symptoms definitely sound like under-treated Lyme & possibly co-infections.
I've been on both types of doxy--I was nauseous, headache-y, and tired on the doxy mono so I asked for a different form. Sadly, the hyclate made me much much more nauseous than the mono. I learned that I couldn't take the hyclate on a less-than-full stomach, and I definitely couldn't take it in with hot liquids, or take a shower or bath soon after taking it. If I did any of these things I would vomit up the meds. Sorry.
The good thing I can say is that despite the side effects of antibiotics, many of my lyme symptoms have cleared or almost disappeared, thank goodness! I hope it works even better for you.
