From what I've read, it's just essentially dead skin that hasn't fallen off yet - an eschar - usually associated with a bite of a Lone Star tick. They say that they normally pass the Rickettsiae and rickettsia-like infections, but I've read others that have Lyme and not any of the rickettsia infections have had this same issue as well. They should continue to watch the bite area for secondary infection (just in case), but not to worry over that black spot.
They need to watch the child for symptoms for quite a while. Most of us have to watch for the rest of our lives. This little one has quite a few years left to live, and a lot of those teenage hormones to live through too!! LOL! And if that's not stressful, boy, I'm not sure what is!
