I got that head pressure all the time during herxes, it sucks.
Azithromycin has very good BBB penetration so it gets into the brain well. Dying bacteria can cause all sorts of toxins to be released and it's kinda pointless to speculate what kind of bacteria is it dying, but it is one that lives inside your brain, and you will feel better after it's dead. What you can do is detox, drink a lot of water with lemon as gfields suggested, melatonin (before bed) is also good for brain herxes because it is a precursor of glutathione (powerful antioxidant) in the brain.
If it becomes unbearable you might want discuss with your LLMD to lower the dose temporarily, there's no point in suffering from herxes, especially brain herxes can damage the brain tissue which as you know it is not known for its powerful regeneration abilities...
