Herxing can be unpredictable, in terms of the when, where, and how. I've seen users report experiencing almost immediate herxing, whereas others have mentioned it taking a couple weeks of being on a certain medication before they had what they would call a herx.
I myself fit into the former category, at least that's been the case with what I've tried thus far - if something is going to work for me, I'll feel it. Though, I should mention that there are things that can potentially keep certain medications from working at the time (coinfections, absorption issues, deficiencies of something, poor immune support, et cetera). So, just because something doesn't work now, doesn't mean it couldn't later on.
Though, if something doesn't seem to be working for you after 3 months, that usually means a change of something is required. Your protocol is missing something.
That said, maybe something is keeping the antibiotics from working as intended? I don't know which infections those particular choices target, but maybe it's not the ones you're most affected by at the moment?
BUT.. you could just be herxing. I forgot to mention that, if the medicine is working, and you keep pumping it into you, it can be a continuous thing. If you're killing the bugs and not keeping up with the toxins, it is not going to just stop.
Reminding me, do you do much to detox? You may need to be doing more. Even if a change of medication is what you need, this should still be a staple of your treatment. Detox.
As for the reflux, are you on any probiotics, gut-related support supplements? Antibiotics can be very hard on the GI system.
Do you take the antibiotics on an empty stomach?
I am not that knowledgeable when it comes to treatment, especially the pharmaceutical side, so I apologize if not much help. I feel like I have heard that doxy is pretty lightweight for Lyme. *shrugs*

edit: is this an LLMD you're seeing? I mean, some of us have to work with what we've got, so whatever doctor that is willing to help is good, but the prilosec + telling you to stop Lyme treatment seems like a novice decision to me.
