As many of you have seen from my other thread, I have been diagnosed with some co-infections and we are starting to go after Bart first.

I was prescribed Rifampin, 600mg once per day. Is anyone else doing a single daily dose? I've read a lot of people do 300mg x2/day, but haven't come across anyone doing one daily dose. Could my prescription be labeled incorrectly?

Also, is the orange urine a possible side effect or should I definitely expect it? Any other side effects that caught you off guard?

Many LLMD's do prescribe 600 mg once daily. And, there are members on this forum doing just that.

I subscribe to LLMD MR (Seattle) - treatlyme online 'book'. He has Rifampin for bart - 600 mg once daily.

I doubt if your prescription is labeled incorrectly.

You should expect the orange urine.

People report being fatigued and also liver enzymes could rise out of range.

I had none of those two side effects.

Suggestion: you do not start with the full dose. 300 mg once daily is even difficult for some.
I took the 300 mg capsule and split it (the best I could) to take 150 mg for several days (I think it was a week)

I did quite well on it. My issues with most antibiotics now are my esophagus - I get some acid reflux after I take them.

I got an increase in pain - nerve pain in my upper thoracic - left shoulder blade area as well as general 'whole body' pain.

I did the 300 mg twice daily.

Thanks Girlie. I am going to ease my way in with 300/mg twice daily and then move up to 600 once daily.

I've taken one pill so far and it's leaving me with a warm feeling in my belly. Kind of odd. Oh well, happy hunting rifampin!

You might want to think about just doing 300 mg once daily for several days.(a week?)..before moving to 300 mg twice daily or 600 once daily.

300 is butt kicking ..twitch increased and yes to orange wee ...it kill bart and cpn and i have both ...maybe myco too ...it works well but llmd only want me on it for 4 month cuz of liver

I had this same question 3 months ago, so I asked my LLMD. She said it didn't matter if you divide the dose or take it at once. 3 months in, and she's increased me to 600mg twice a day.

I get the orange pee as well.

That's a high dose, mustard seed. Are you not getting results (i.e. herxing and/or improvement?) on the 600 mg daily dose?

@girlie im trying to work it in my schedule like you and reminder. .hope i tolerate it for a bit ...chest pain shin splints on forearms ..etc etc

Sorry for the late replies.

Girlie, I have yet to see any herxing or improvement. I have only been on the higher dose for a week. I did 3 months at half that dose first. I did start getting pain at the bottom of my feet recently. I don't know if it's bart or just a coincidence.

Blue, I do a liver panel and a bunch of other blood work every month. Everything's been fine so far, except for low neutrophils.

I think getting the pain at the bottom of your feet is a good sign that the meds are hitting the bart infection. I mean, I'm no expert and I don't know your full history, but it could be a great sign :) fingers crossed for you!
