I think the best llmds/llnds will generally have good strategies for this infection, they usually understand these infections stretch much further than just lyme and that lyme is not always the key driving infection (for instance I think bartonella is my driving infection and it is hard as heck to knock out).

There are many others, but I have high respect for Breitschwerdt (he does not see patients from what I understand). Dr M does see patients, he seems pretty good but I think there are others that are pushing the envelope a bit more (at least in their attempts to resolve the trickier cases).
I think PeteZa gives good advice, research llmds in your area or llmds you are interested in seeing, contact them to see what kind of experience and strategies they would be proposing for your case/bartonella and if these strategies make sense to you then that would be a good person to work with! I would also want to know if they have contingencies if one set of therapies doesn't do the trick, what is their gameplan and how conservative or out of the box are they in their approach?
