You didn't mention what your herxing symptoms were, so that might make a difference in what you try. The idea isn't to herx to an unbearable level. Some people don't herx at all. But in your situation, you received some good info from trying those components that you reacted to and it might be good to explore them again.
I agree with goshawk - I would start including the oil of oregano, monolaurin, CSA and OLE but with a few strategies in place:
Before starting with anything, goshawk also mentions adding in a good GI binder. This is a big chunk missing from your current protocol, which is outstanding, btw. We take almost everything orally and this results in a lot of added burden to the GI and immune system (70% of it is in our GI). It's important to assist GI detoxing.
Then, I would slowly integrate one of the above 4 components, slowly work up to an almost an optimal dose and VERY manageable herxing, before adding in another component and slowly work the new component up to almost optimal doses while maintaining levels of everything else. Do not increase or add until the herxing is manageable. Increasing detoxing as much as you can along the way.
I also agree with Hoagie - while exercising is good, it is also creating a huge burden on your body to supply it and the recovery process with energy and resources. It's likely causing the exhaustion and can also cause damage. I'd look up "exercise for Lyme" and follow recommendations, which include not to do it two days in a row, no aerobic exercise, and certainly not 2 hrs at a time. In fact, while you are incorporating the new antimicrobial components, I would not do any exercising, allow the inactive detoxing to do the work for your body and conserve your energy.
Also -
The LDN isn't exactly "for Lyme" - it's an immune booster as well as a calming mechanism for the nervous system so it can help overall and certainly with some neuropathy from other infections.
