Hello everybody,
I am on Long Term disability from my employer for last 8 years, and every year I was getting
Activities and Financial inquiry for me and supplemental assesment form for my doctor from my LTD insurance. This year I did't get one.
my question is: should I call my LTD insurance and ask about that?
May be they sent it and it got lost in mail.
I have only 30 day to respond on it until they cut my benefit.
