Best antibiotics that cross blood-brain barrier?

What are the most effective oral antibiotics that cross the BBB and get into the brain and CNS?

Tetracycline actually gets into brain better than mino. Mino is smaller in size than tetra but you can take 1500 mg a day of it compared to smaller mino dosage, so more gets into brain. Both dr donta and sp in ct use it but most use mino for some reason

Mino reaches MIC faster than doxy and tetra at 100 mg two times a day.

Dr Donta was named Dr tetra because he rarelly prescribed doxy or mino... I think he was wrong when he prescribed biaxin (don't cross bbb) to treat lyme, because he says that borrellia affects blood vessels that supply CNS and other organs... It's true, lyme affects blood vessels, cause vasospasms etc.... But can affect neurons etc... And entre into cns in 24h after been infected. He's an old school but miss a lot of conceptos.

Currently, treatment of tuberculous meningitis involves the same drugs and doses as other forms of tuberculosis [5–7]. While isoniazid and pyrazinamide have good cerebrospinal fluid (CSF) penetration, rifampicin concentration in CSF may not reach the minimal inhibitory concentration for tuberculosis, and ethambutol and streptomycin have poor CSF penetration [2, 8]. Among second line drugs, levofloxacin, ethionamide and cycloserine have good penetration in CSF [8–10]. In a phase 2 randomized controlled trial investigating the safety of moxifloxacin and a higher intravenous dose of rifampicin during the first two weeks of treatment of tuberculous meningitis, the use of a higher dose of rifampicin was associated with a survival benefit [11]. These data suggest that increasing the CSF penetration of the initial treatment of tuberculous meningitis could potentially reduce its mortality.

I think rifampin only penetrante BBB with a high peak in blood. It has a big molecular size 888, but penetrate good in lipids... for that reason 600 in a single dossage is better than 300mg twice day.

Pyrazinamide is usefull in treat Bartonella? I've read about it in horowitz article... It has excellent cns penetration like dapsone.

What about Mepron? I've not read anything about it crossing the b/b barrier...but when I was taking it the first time around...I felt it in my head - increase in head press.

It's molecular size is little 336 more or less... It penetrantes good like malarone.

Malarone has always given me the most wicked head herxes, similar to what Girlie mentioned she gets with Mepron, terrible head pressure, vicelike headache. So that may be because it has good bbb penetration?
