If you have strong herx responses and flares to antimicrobials I think the 2nd doctor's approach will help in tolerating treatments. I've found LDN helpful, magnesium threonate is also a good one. The antihistamines may help with the die-off and cytokine cascades if this is a problem for you.
In regards to the antibiotics in the protocols it depends, I think both protocols can be effective. But each patient responds differently, one important factor is that if you plateau and still have symptoms it's important to rotate the meds.
I can say is that given my huge sensitivity and issues with massive herxing I would do better with the 2nd doctor's protocol. But that won't be the case for everyone!
