I know you have posted about these questions before, and I hope you find helpful answers soon.
To me, everything you describe sounds like it's adrenal in nature - afternoon fatigue, low-grade fever, depression, hair loss, weight changes, etc. Of course infections can throw off your HPA axis, but to some degree herxing and stress can worsen the adrenal issues, so some LLMDs will recommend treating HPA axis dysfunction during or even before treatment for infections. EBV is especially gnarly for causing adrenal dysfunction. Have you been tested for all types of hormone levels (thyroid, adrenal, pituitary, etc.) at various points in time? Even a small imbalance can cause symptoms. Are you taking any adaptogens or doing anything else specifically to balance the HPA axis (ensuring sleep, meditation, bioidentical hormones, no stimulants, etc.)?
Hope you get to the bottom of this! I know how frustrating these symptoms are.
