Recurring E. coli in urine, possible leaky gut??

My mom has had recurring E. coli infections in her urine and microscopic blood sometimes as well. Don't have the full results yet for the latest test, but assuming it will be the same as last time. Her family doctor has given her Macrobid, it clears it up for a while and then infection returns. She is on lyme herbals from the LLND. I am wondering if these infections are from a possible leaky gut? Has anyone else ever had this with Lyme? Thank you.
