For whatever reason its known to work synergisticaly with other abx. Thats why "plaquenil + macrolide" is a big combo. I'm not sure what effect it will have on its own on a bacteria, but I would assume it wouldnt be

W/ respec to MDL, i think theyre very good. Thats the lab i trust the most. Dr K also uses them, and if he trusts them its good enough for me

I would be very surprised if you had RA w/o a positive ANA or any other antibodies. Maybe some form of infectious/post-infectious autoinflammatory disorder, but then your ESR would likely, although not necessarily, be high. Its all very odd. Although i do see people in the AI boards who have exactly this presentation all the time. Borderline ANA, normal labs, but clearly inflammatory symptoms. Unfortunately there are immune mechanisms we just dont fully understand yet, particularly inflammatory processes, and antibodies we havent discovered. Do NSAIDs help the joint pain? Or can you not take them because of your kidney issues?
I think a trial of plaquenil again makes sense.
