dcd: My guess (with fasting at least) is that if you're starting from somewhat of a blank slate, rebuilding balanced flora in the gut is easier to do than trying to treat a gut that is already imbalanced.
Rebuilding can be easily influenced by eating habits/diet.. and I also believe genes and other natural influences promote a proper balance for each individual.

Have you considered fasting again? I remember you tried a 5-day water only fast (kinda on the fly) and you said it gave you temporary relief...
Good call POTs, I'm guessing that's it as well. I guess what bothers me is most blogs you read will mention limiting carbs to cause die-off or using antimicrobials to heal SIBO, yet that's only one step. You have to rebuild the microbiome as well w/ pro/pre-biotics. And this has to be done in the right order. Taking probiotics when you have a SIBO problem is counter-productive, until you get the problem under control. But I rarely see this discussed, kinda being forced to piece it together.

re-introducing fasting. I had a great experience last time I fasted, in fact january when i fasted and then ate really clean was the best i've felt since my neuropathy began. Then i got the serum sickness when i turned my IVIG run-rate up, and covid hit, my diet degenerated and it was all downhill from there.
I keep coming back to this microbiome thing recently. My stomach just feels so rough, like there's inflammation or acid down there. And I've spoken before about
how ASG calms my neuropathy. Makes me think I've got massive amounts of inflammation down there. What I know is that my neuropathy began after a bout w/ SIBO. What I also know is that I feel better when I eat well. ASG helps. I also know I've had fatigue and digestion issues my whole life. All seems to come back to my gut.
I've been working under the assumption that I had some sort of infection as a kid, lyme or what not, that was causing my fatigue and iritability. What if, I just had smouldering inflammation coming from my microbiome. And eventually it just blew. I found the article below pretty thought-provoking at describing how the microbiome can screw up the immune system and eventually trigger autoimmunity. All just theories, and how do you truly rewrite your gut. Sounds like when youre born by c-section, which I was (and actually quite a traumatic one.The anesthesia didnt work, my mom felt them cutting, they had to pull me out when she started screaming, pushed in my skull, had neurosurgery my 1st 12 hours of life and ended up in the ICU...related?), and then eat a crap western diet like i did, you just have a screwed up microbiome, and run the risk of chronic sickness. Although in that article they mention how simply cutting out the western diet for even 24hrs leads to noticeable differences in the gut bacteria in mice. Maybe there is hope. But then I see so many people who got sick while eating clean, or who go paleo for a year, eat strict diet, and it doesnt help...and I come back full circle in my thinking, no point trying, once you've got the problem its there, pharma is the only way out (rituxan, IVIG, cellcept, Efgartigimod), immunomodulation by Rx is the only thing that will get it under control for me. Very frustrating these chronic diseases
