Rifabutin and azithromycin which one should I start first

Im starting rifabutin and azithromycin as I was previously on rifampin and minocycline. Should I start with the rifabutin or Azithromycin first? I wanted to start azithromycin first but I am waiting on my doctor as I was concerned about azithromycin associated long qt syndrome. So should I just take the rifabutin first? I am suspecting a bad herx from this so I kinda wanted to get it over with. Im thinking the minocycline might be resistant so I don't know if im on rifabutin+ minocycline, then the rifabutin could become resistant as well which I do not want happening. How long does it take for a drug to become resistant?. Im confused on what to do and very open to suggestions!!

Yes I am taking rifampin + mino and replacing with rifabutin+ Azithromycin. I am replacing 1 at a time and decided to go with rifabutin first. So for a brief period I will be on rifabutin + minocycline, then putting the Azithromycin in to make it it rifabutin + azithromycin. I started with 150 mg rifabutin, how soon should I titrate up to 300 mg? Ive heard some suggestions to take it slow
