firstly everyone has both white and grey matter in their brain

the white "hyper intensities" ie bright patches on an MRI - are something different - they just correspond with areas of the brain that are responding differently to the magnetic resonance of the scan than other areas.

most typically these are caused by changes in the myelation ( fatty covering that helps the nerve function) of the nerves in those regions, which in turn is most typically caused by changes in the small blood vessels supplying those nerves.

this is a common finding in lyme disease and co-infections - that are known to cause changes in the small blood vessels in the body and inflammation in the central nervous system.
its also a common finding in normal aging and other inflammatory conditions in the CNS

its not really something to be super concerned about - a small amount of hyperintensities is not an indication of a serious problem - changes like this are to be expected in any disease that impact s the CNS - but they are also reversible with treatment

the treatment is largely just treating the causative infections
so - in other words - doing what you would be doing anyway

the reaction to antibiotics / changing to different antibiotic can often cause a herxheimer reaction that makes symptoms much worse for a time

exactly how long varies from person to person and treatment to treatment - normally things start to calm down after a week - but it can take longer if the herx is severe - or your system is generally hypersensitive or dysregulated - or if for instance as you hint at - other elements in your life, like stress, trauma, sleep issues etc are causing your system to be more reactive than normal

once you have a positive lyme diagnosis - further testing with western blots / immunoblots will not tell you if lyme is present or gone - or what the disease is doing - as a person can continue to have elevated antibodies for years after the disease has resolved

bartonella is also associated with small vessel damage - and it would therefore probably be wise to discuss targeting that along with lyme in your treatment approach ( there is a lot of overlap of active antibiotics - so this is not too difficult to do)
