Vag/anal fistula

Hi I’m a Crohns patient 13 years. Had one rectal fistula that was surgically repaired 3 times all eventually failing but Remicade closed it. 3 years later now I have an entirely new fistula running parallel from top of rectum to about half way up the left vulva. I have pain and bleeding constantly. Waiting to start Stelara. Can barely sit anywhere and riding in the car causes a great deal of bleeding. Wearing diaper constantly and the long space between the openings where it bleeds from is very distinct. How long can I go on bleeding like this? As it is most of what I do is sleep or sit for short periods. Can barely work part time from home. Any suggestions?
