Can some one please tell me who has seen this doc duke bahn
What the procedure would be, I am very close to making an
Appointemt to see him, the question I have is I will be traveling from Alabama
If he does the Doppler color ultrasound and sees nothing suspicious
He does not automatically do a biopsy, but if he sees something suspicious
He does biopsy, then when I leave and go back to Alabama will
The oncologist in Alabama go by his information to treat me, or will he have to go through a similar procedure, or does anyone know any one in
Southeast that would be good at using Doppler color ultrasound

Dr Bahn will do a biopsy on any suspicious area. He will call you in a week to discuss your results and will send you the biopsy pathology report from a local lab. He will also send you a letter outlining his recommendations on treatment based on your biopsy and what he saw on the color doppler. He will also give you a copy of the color doppler images and notes and drawings indicating what he saw.

You can bring these to any other doctor anywhere in the country for a 2nd opinion and to discuss treatment options.

Tiger, What is the reason that you want to see Dr Bahn? Have you had previous biopsies? Please tell us more about your situation.

John t, I have never had a biopsy, a local urologist recommended
A biopsy, i have had a rising psa level to 3.8 from 3.0
My free psa was 20, I have crohns disease, I am trying to
Avoid just having what I call a random biopsy, I like the
Idea of the Doppler color ultrasound, what I am hoping is if he does
The Doppler and sees no suspicious area he will forego biopsy,
I understand with biopsy u will need to take anti biotics and having crohns
Might cause me to flare if I take anti biotics, but from what I read on
Forum he seems to be the best, any suggestions would be appreciated
I tried emailing u John t
But it came back undeliverable

I believe you might be closer to Dr Fred Lee in Mi, who was Dr Bahns teacher.....When my husband saw Dr Lee last June,he was only in the office 1 to 2 days a week......my husband was trying to avoid a random, unnessary biopsy [rising psa]......he did color doppler [with option to biopsy if nessary] didn't see anything,prescribed antibiotic for 4 wks, [psa went down 3 pts]did color again and did not see any suspious areas to biopsy. Dr Lee was very thorough and we were very reassured by his evaluation......Google, Rochester Urology in Mi for contact information......hope this helps......Cooper  wink

With a dropping PSA and Crohns disease, I would stay local and seek the advise of an oncologist (Dr Windsor at UAB?). PSA is not a perfect science and even if positive it is showing signs that it may be a weak variety of the disease. Spending the money to travel for this test, which is also imperfect (do you remember the Duke Bahn interview I linked where Dr. Bahn also states that it is imperfect).

First and foremost do not overreact. The standard biopsy procedure is not very far from a guided one with CDU. In fact in 15% of cases in a controlled study, a standard biopsy detected a cancer where a guided CDU did not. But in any case, with Crohns you have a reason to play the game slowly and check for doubling rates. There is another test to add and that is the PCA3 test. This is a simple urine test that combined with the others may help with this decision making process.

I have little understanding whether Crohns can exacerbate these test results but it is worth looking into with a qualified oncologist before the long travel plans.

How long has your PSA been elevated?

Actually my psa rose from 3.0 to 3.8
I guess in my post it was confusing the way I worded.
I guess now I am confused on what avenue to take,
How do you know about this dr Windsor at uab
Have you met him, or do you patients that have used him

Just some additional info as I have been to see Dr. Bahn twice for color doppler testing.

After both color doppler tests, Dr. Bahn decided that a biopsy was not needed in my case. He will only proceed with a biopsy if needed and will only take a few samples as the color doppler lets him target in to the exact area.

I believe that Dattoli in Sarasota, FL also uses color doppler ultrasound. That may also be closer than California but if it is the specific doctor's recommendation you are after then this would not be of any help.

I don't know that travelling for a 1st biopsy is worth it. Getting a CDU for a 2nd biopsy or for staging is well worthwhile, but for an ordinary biopsy it may not be worth it. Most other CDU operators will take a random sampling even if they don't see anything. Bahn will not. I guess it's your call.

Very confusing, I am trying to avoid biopsy if not necessary,
Just a poke in the dark for random biopsy, and comes back negative
Sounds like still not a lot of satisfaction, also side effects
Do to biopsy, does not appeal to me, okay let me change
Direction here, if u were to have first biopsy, and wanted to go
To one of the top specialist in country where would u go.
Let's say in southeast.

In the southeast United States, I might talk with Dr Pow-Sang at Moffitt in Tampa or Dr. Vipul Patel in Celebration, FL. Of course, there are many oher good doctors in the southeast, so those are just two possibiliies. Best wishes.
