My husband was also diagnosed at PT3B on pathology (see signature below). Surgery was Oct 2013, radiation about 9 months later (waited for continence) and increasing PSA since then. His care was transferred from the radiation oncologist at Moffitt Cancer Center in Tampa (where we live) to the medical oncologist at the same center once it was obvious that the radiation was ineffective. The medical oncologist felt John's cancer was "incurable, micrometastatic" and that he would need HT at some point. Here's where it gets interesting. The oncologist felt that when the PSA hits 1.0 or greater, a bone scan would be in order. But he would wait for the PSA to go to 8-10 before starting HT. We decided to get a second opinion and saw Dr. Slovin, a medical oncologist with Memorial Sloan Kettering. She concurred with our oncologist in Tampa, also stating that she would not start HT til PSA increased to 8-10. Her only difference in treatment was that she would start with Casodex instead of Lupron. We met our local onco again and explained what MSK doc said, he agreed with her use of Casodex and also stated he would do intermittent therapy (as did Dr Slovin). So here we are. Every 3 months a PSA, bone scan when it hits 1 (which we are anticipating to be early next year) and HT when PSA is 8-10. One other thing our med onco mentioned was when it gets to about 2, it'll double quite rapidly so getting to 8-10 could happen rather quickly. Hope this information helps.
