No apologies needed. I am sorry that you have to deal with this. The T refers to to prostate, N to the pelvic lymph nodes and M for organs farther away from the prostate and pelvic area. The higher the number the more agressive the cancer.

T3b means that the cancer has spread past the prostate and into the seminal vesicles.

N4 means extensive spread of the cancer in that pelvic lymph node area.

M1b means that it has spread past the pelvic area to the bone and perhaps other organs.

It is Stage 4 D2 cancer. That said there is still reasons for hope. There are a lot of treatments to minimize the effects and spread of the cancer. A lot of men here have similar cancers and they are still here.

As pointed out, the N refers to the lymph nodes affected. My very non-medical guess is that it doesn't make a significant difference in that you're working for systemic control (and getting it based on the initial PSA response).

Juxta: means near to
regional nodes: means lymph nodes that drains lymph from the region around a tumor.

If it is even further away from the tumor it counts a metastasis.
