Just disgnosed 54 yrs old. thoughts on what to do

tmichael - Your stats aren't all that different than mine. I'm older, though, so that may affect my choice among the options. I was told that I would qualify for one or more of the AS protocols, but since I had one core that was read at G3+4, I opted for action now. Maybe, had I pursued it, I might have had my biopsy downgraded to G3+3, but that will never be known now.

I opted for CyberKnife SBRT radiation treatment - one of the ones the guys have suggested you look into. I can tell you that by and large, it is not an imposition on my life. The whole process went smoothly and quickly. I am only about 3 weeks out from my final treatment, and my few immediate side effects have just about resolved. I will admit, though, that we won't know the final situation regarding things like ED for a while yet, but I am confident that all other aspects of life will/have quickly returned to normal. I am also confident, based upon my biopsy report and the studies of the efficacy of SBRT for low-risk prostate cancer that I will have good results. I won't be looking back to question my decision.

That said, if I did not have the G3+4 in my biopsy, I might have elected AS. It just makes too much sense.

my wife gave it to me. to late for me. i had brachy over a year ago and my psa last time under 1. doc says in 2 -3 years should be near zero.

Thank you to everyone for the advise. I did go ahead and have the surgery based on a lot of factors. I want to say that Dr. Ghazi did go over all the options and I don't feel that he pushed me into the surgery. He did say that if I was 10 years older, he would recommend doing nothing. I did not know at the time if I was making the right decision or not.

It turned out that the pathology after the prostate was removed was worse then the biopsy showed. The Gleason was 4+3 and the cancer was 30%. Dr Ghazi said that the removal was going to be inevitable and by doing it now instead of waiting another year for the PSA and biopsy to show the change, that it was still contained in the prostate and was able to spare the nerves. I guess the decision would have been made for me in 6-12 months anyways.

Thanks for circling back around. That's quite an upgrade with the pathology! I'll echo what tennis player said and wish you a speedy recovery and a lifetime of 0 PSAs! All the best from the guys with the similar screen name.

Good for you to remove it. Removing the prostate and having the pathological assessment of the tumor is still the only treatment that will report the full and complete extent of one's cancer. Inform us how your recovery is going.
