My MRI results showed something similar. The wording was something along the lines of "bulging appearance suggests extracapsular extension". Irregular borders and bulging, to my understanding, are suggestive of that. Also said suspicious for neurovascular bundle involvement. CT is typically less clear than MRI, so if they're seeing it on CT there's a good chance there's something there.
Surgery was offered to me, but I was warned it would have to be a "wide excision, without 'luxury things' like nerve sparing". The side effects were going to be at the worse end of the risk spectrum. My uro was quite equivocal about success with surgery, for me. Maybe ok, but just maybe.
I ended up choosing IGRT by IMRT with 3 years of HT (long, not usually done that long now, but it was because of the G9 5+4, and the probable cT3a staging).
Good luck with the decisions. Nothing is easy or clear with this disease and its treatments.
