This whole subject is of interest to me. Months before my one and only biopsy (which led to my diagnosis earlier this year), I had expressed concern to my urologist that my testicles both seemed notably swollen. He didn't seem the slightest bit interested or concerned at the time, and my PSA was heading up rapidly. Upon my insistence, he ordered an ultrasound of the scrotum and testicles, which came back negative, and he later stated that he felt the swelling was simply general pooling of fluids in the lower body and limbs from insufficient exercise and poor circulation. Weeks later, the biopsy led to a high risk PCa diagnosis.
Interestingly, the testicles both remained swollen until a couple of weeks into recent Casodex treatment, when the swelling seemed to dissipate considerably, and by chance, the PSA (not T), has since plummeted. Now, I'm learning here that ADT in-general is known to shrink them.
A guy hates to jump to conclusions, and maybe everyone else on earth already knew this, but there certainly does appear to be a correlation of sorts here, wherein swelling is proportional to PSA and or T levels.
