Likely your brother should be more agressive about his treatment and you at G6 with your PSA history should continue on AS for awhile - that's not over treatment. That's prudent.

The difference between you is the gleason score. A G6 puts you in the low risk group, while your brother likely is a slightly higher risk group. You two also seem to have different attitudes towards treatment and health management. You can't change your brother.

But you can keep looking out for you.

I have another question for anyone about the biopsy. When I had my first one seven years ago there was not any pain prevention done, just an antibiotic, and it was painful. Since that time I have read that convincing a patient to have a second biopsy is more difficult than convincing them to get the first one, which is where I am at now. While searching the web I have read about prostatic blocks. Has anybody had one of these blocks and do they work? A urologist on prostatediaries.com said that for his own biopsy, the block did not work.

Also, wouldn’t the needle delivering the prostatic block hurt just as much as the biopsy needle?

I've had two biopsies, and the block worked for me both times. I don't have a basis for comparing the pain from the shot for the block with the pain from a biopsy needle puncturing an unblocked prostate. However it makes sense to me that the needle for the shot would be finer, and that a biopsy needle removing a core of prostate would cause more damage and likely hurt more. And there's just one shot for the block, as opposed to a dozen or so biopsy needles.

I have tried to find the minimum number of cores needed to calculate a Gleason score but couldn't find an answer anywhere. Does anybody know what the minimum number of cores is to calculate a Gleason score?

a score is assigned to each core taken. The G scores generally reported here is the highest G score from all the samples. Not all cores will have PC, so the number of cores with/without PC is significant.
