I will be starting my IGRT,IMRT/VMAT +ADT soon. Sine I did not have surgery prior to EBRT..I’ve read that I am at a Higher risk for secondary cancers after radiation. Has anyone used the “ spaceoar” to counter this risk. what are the pros and cons?

The secondary cancer risk from radiation is minimal. Studies show it is about
equal to the risk of dying from surgery for prostate removal....less than 1%. A recent European study of over 350 men put the long term occurrence at 1/3 of 1% (0.3%). Remember that the radiation risk is more associated with the techniques of 15-20 years ago before the more modern IMRT/IGRT technology in use today. These better methods will no doubt lessen long term risk as they minimize radiation to healthy tissue when compared to older methods. Also, it takes easily 10-15 years before radiation induced secondary cancers can be measured.

Combination treatment of HDRBT and IMRT. This combination works better for high risk than IMRT alone. Without the HDRBT I would have had about
40 IMRT fractions rather than the 25 I had in the combination treatment.

I had the 'Spaceoar' injected between the prostate and the colon prior to my last proton beam radiation treatment. Since I had already gone through IMRT radiation thearapy six years prior, there was some question whether the 'Spaceoar' could actually be used due to the previously radiated tissues becoming more rigid. It was worth the effort though since the PB radiation could be delivered at a higher dose more simply.
I will not lie,though, it was a very painful experience trying to get the 'Spaceoar' into place with rigid prostate tissue next to my colon. Maybe not so for those who have NOT previously been radiated.

I just completed HDR brachy boost. My RO convinced me to allow the SpaceOr gel spacer. He said it enables him to get more dosage in the area in which it is positioned.
