Yesterday, I had my markers and Space OAR appointment. I didn’t know it would be a musical chairs event. I was given 1 Oxy and a 2mg Ativan pill. I was taken into a room where my nurse liberally applied Lidocaine to my peritoneum. Then to another room to watch a CD on IMRT. ( learned it already on this forum). Taken to another room where I hopped onto a high legged saddle. The RO came in said a few jokes and went to work by saying here comes the probe. It was a little more intense than the biopsy because he kept turning it it around to get the best picture. Next was the long needle to place the markers and a double syringe to place the Space OAR. He liked what he saw and so I was taken to another room for the scan and tattoos. A small catheter was inserted for dye and the scan. No pain at any point. I was given a sucker and sent home. No after effects today. Hope this helps anyone going for this procedure not done under anesthesia.

An alternative method, if interested?

Had Fiducials placed (4 gold markers), 2/1/18. Had to do a low fiber/residue diet, 2 days
before. Also, had to take a laxative 6 pm night before and restrict food/water intake at
midnight. Also, had to do 2 fleet enemas 1 hr. before leaving.

Was given Propofol and inserted an ultrasound device, in the rectum for viewing the
placement, and a catheter (smaller in dia. than the foley we had after RALP surgery) into
the bladder. Fortunately, this was all done while I was in dreamland and didn't feel a
thing! 2 markers were put even with the L4 Vertebrate (where Prostate was) and the
remaining 2 in the Prostate bed.

My guess is each treatment team has different protocols. I was treated with SBRT more than four years ago. At that time the markers were placed a few weeks ahead of my simulation. Just a local anesthetic and TRUS guided placement not much different than my biopsy. A non event. SpaceOAR was not widely accepted or used at that time although intuitively seems to have value. John and Terry, thanks for your input. I also had the diet restriction ,clean out the night before and morning of. John, I asked several times to be put out and was told it was not needed. I also agree that different ROs in different settings use different techniques. My OR is in a private practice.
A short history: Some blood found in semen in 2015, URO said there was nothing to be concerned about. One year later, still getting blood in semen. I asked to go to a new URO. The new URO said it was not normal. First biopsy showed 3+4. PSA 2.5 . TURP performed for urinary issues and PSA .05 have been watching PSAs until another URO did second biopsy a few months ago, Gleason 8. Now it is scramble to stay ahead of it. Be your own advocate. I trusted my doctors and knew nothing about PC. The best to all.

I was put under general but light with profolol. My only issue was the male nurse manhandled my left leg to get into the stirrups and I had told them I had severe hip, leg and back pain. He pulled it sideways and with a lot of force and I almost jumped off the table. I remained calm because I did not want to offend the people who were about to jab large bore needles into my sensitive area but I wanted to knock him on his butt! (maybe in my old days LOL) But I was really ticked off since I made a point of my pain condition.
Then? Ah sweet "La-La Land" and I woke up in recovery with very little pain. The nurse said she could give me hydromorphone and I laughed and say "go ahead but it will not do a thing" She injected it into the IV and nope! Nothing!
I take buprenorphine for my pain and it clings to opiate receptor sites and one would need massive amounts of pure Mu agonist opioid medication to even be noticed (just a side FYI)

No pain afterwards just a little sore and I felt like I was just a little wealthy with all that gold in my butt area!

Swan, sorry to hear about the rough handling. I have had three back surgeries with two of them being fusions to the lower lumbar. As far as I know, if I wouldn’t have told them the pain I get while lying on my back, I would have had just Lidocaine. The female nurse treated me with care and took her time getting my legs up. The meds did help. Given the price of gold and the cost of treatments, maybe we could just tell them to give us the gold and use something cheaper. Given the size of the pellets we might be able to purchase a double shot of some strong beverage and skip the pain meds.
