I just took my three month post RP PSA test, and now the brief wait for my results. My previous, and only, post RP reading was 0.017, six weeks ago.

I questioned Your Friend's path report when he posted a more complete version. If he was "pT2 with negative margins," there shouldn't be any "focal" EPE. I think he should ask his uro to explain how the word is being used in the report.

Negative margins mean that at the outer edge of prostate a small amount of non cancerous cells were observed. Negative focal means that a small tissue sample was also non cancerous. In other words all good. That I found on the web. My best to you and hopes that upcoming results are just as positive.

Regarding my +focal:
There was a 2.8mm invasive carcinoma unifocal margin not identified in the EPE, of Gleason pattern 4. So, I am classified as Negative Margins + Focal.

Does the staging "pT2" appear in the report? I do not understand the sentence as written, other than that there was a positive margin limited to one spot ("focal") and there also was EPE.

Does "Negative Margins + Focal" appear in the report or are they your words?

Yes, pT2 and negative margins + focal both appear in the pathology report as I've written. I don't believe I wrote there also was EPE.

I misread what you posted, I thought you had both negative margins and focal extension. In any case, I agree your URO needs to explain what was found in greater detail. As always with a question I went searching for an answer. I did look at several studies. I found that BCR in those with post surgical negative margins and a Gleason score of 7 or higher were at a greater risk at the 5-10 year mark than lower GLs. However it looked like the difference was only around 10% or less. It would seem that no matter how great the surgeon or technique the chance for reoccurrence is still there. I would think that radiation only or with ADT is the same depending on GL score. Looking forward to an undetectable result for you and many great years ahead.

Excellent. That puts you in the "nadir below 0.010" group, which is even better than the "nadir between 0.010 and 0.030" group" for risk of BCR!

Friend, that is as good as it gets. Now get used to doing it every 6 months for the next 5 years. Party time!

Remember that trifecta is: Undetectable PSA, Continence, no ED. How's your trifecta score?

I'd say it's good to excellent. Incontinence was never an issue. With ED, I have something that's usable, but not up to pre RP standards. I may recover more with time.

Today was my office visit to discuss my latest PSA, and to take a Uroflow test. My bladder was measured to contain 400 MLS before urinating, and was measured to have voided completely after urination. My uroflow score was 40 ml/sec. A typical man's range is 10-21 ml/sec. I'm peeing like a race horse! I'm thinking about changing my forum name to SeaBiscuit.
