Small blood clot after orgasm?

I guess this question is for both RP and RA treatments. Over the last half year, after orgasm, the first time urinating I have a small blood clot come out every time. I do not have a clot in any further urinations. I have mentioned this to my Uro and he said there is nothing to be concerned with. My first Uro said the same thing when I told him I had blood in my semen. One year later I had PC. I did have a TURP many years ago, but I would think that would have completely healed. Should I be concerned or is this normal after RA after two years or RP?

Jack, that is my story as well.
Uro said blood in semen means nothing. Just something that happens from time to time in old men.
BS! After waiting too long to change Uro's I was G8 (or G9 according to J Hopkins).

Hopefully others will be helped by reading these experiences.

Since you still have a prostate, and all the plumbing is as it always was, I'd be somewhat concerned as well. As an RALP guy, if that happened I'd be more than a little concerned, I'd immediately start thinking "Bladder".

I experienced, as I was told to expect, blood in semen for about a month following radiotherapy. It was not in clots, rather it turned the fluid dark brown. Never anything abnormal since then.

To be more clear on this topic, I had a TURP back in 2017 for BPH. Since that procedure I have had no ejaculate. I have had nothing but dry orgasms since then. I also had some minor bleeding like you after the RA treatment which resolved quickly. Over the last several months after a dry orgasm, the first urination has a small blood clot with it. Any other time urinating there is no clot. I do have a Uro appointment coming up soon. I will ask him again if this is normal. I would think something could be done to see where the bleeding is coming from. My PSA is creeping up, but hopefully that could be the two year bounce.

Thanks for the clarification. So you had blood in the semen before the TURP, then no ejaculate, but no blood. Then ADT and IMRT. Then some three years later blood clot in urine after a dry orgasm. Do I have the sequence down? Sounds strange to me.

I’m concerned that your uro isn’t concerned as we are always told that blood from any orifice isn’t usually a good sign, particularly when it becomes routine. Every uro visit since my treatment always starts with “How are you feeling? Any bony pain anywhere? Are you peeing and pooping OK? Any blood anywhere?”

Probably time for the dreaded cystoscopy. I’m guessing that you have a weak blood vessel in the urethra passing through the prostate, or perhaps in a seminal vesicle. This is then irritated by the orgasm action and leaks a drop of blood. With a little time the blood drop forms a clot that is expressed in the next urination. If that is all, then probably not a big deal. There is a condition called seminal vesiculitis that can express blood in the semen. Hal’s bladder concern, seems like that would present blood regardless of an orgasm. Certainly hope you get a resolution at the uro visit.

Your sequencing is spot on. Thanks for your thoughts of what could be going on. When I see my Uro again, I will press him harder on what is happening and if there is any treatment needed. It would have been great if he took the time, as you just did, to tell me the options of what might be going on. Most doctor visits are like cattle calls. Get to the office 15 minutes early, then a 30 minute or more wait to be called back to the exam room. Another wait for the doctor to show, then a few questions and you are shown the exit.

I've had greenlight laser and then SBRT and got a stricture a year after treatment.
Balloon dilation so far has been working great however hematuria and blood in semen is quite common for me.
I no longer worry about it.
BTW lastest PSA down from 0.9 to 0.69
Small drop but I'll take it.
