I read these as separate, the colon being typo for a period: (1) LVI couldn't be determined (I don't know why not) and (2) PNI might be "focally present" (spots of it may have been present). You can ask your doc about
the LVI, and, if necessary, have him check with the pathologist. LVI would be an added negative finding.
You can check the small print on your Decipher report, but it used to say that a Decipher Biopsy score matches a Decipher RP score about
70% of the time. However, I didn't remember you had the Decipher Biopsy test done and I agree with you: the cases where it doesn't match are probably those that go from a
lower to higher score because the biopsy didn't happen to hit any higher grade lesions that were present. I would doubt scores go down.
