If you are not in LA then I would still contact them as there may be recommend something closer to you.
If you do have treatment then find the best that makes sense to you. I think there is a big difference in outcomes.
Prostate cancer kills. My understanding is that HT can have lots of problems. I think a really good active surveillance program is aimed at avoiding death, HT, and the treatment when reasonable.
