Casodex is a form of hormone therapy that can slow down the growth of the cancer but eventually it stops working and can even make the cancer grow faster. Pain relief is not its primary purpose. There are other drugs that should be used for pain relief.
There are advanced forms of hormone therapy and chemotherapy that can provide relief even after casodex starts working. There are drugs that directly target the cancer that is in the bones and causing the pain. And there are strong pain killers that can help him feel better even if the cancer cannot be stopped.
It sounds like his local doctors are not able to give him the most modern and powerful treatments, which include both drugs and radiation. If it is possible to get him to a modern cancer hospital he would get a lot of benefit from it, although it is no longer likely that the cancer can be completely cured. He could have months or years of comfortable pain-free living ahead if he is properly treated. It is best if he can see a medical oncologist who specializes in prostate cancer but that may not be an option in Senegal.
