I came across this article about giving PCa guys high doses of testosterone. Just the opposite of everything I have been told. Interesting article however don't think I would want to participate in this.

I have a standing agreement with my oncologist that in the event I recur we will seriously consider Bipolar Androgen Therapy. I've seen the Compassionate Oncology white paper before. He's been one of a few lonely voices crying in the wilderness, waiting years for the science to turn around. It seems to be happening.

Here's a more-recent research paper that lays out some of the theory and the evidence. It is impressive but not an easy read.

Adaptive auto-regulation of androgen receptor provides a paradigm shifting rationale for bipolar androgen therapy (BAT) for castrate resistant human prostate cancer.

Thanks for the response, I new somebody out there had some knowledge of this type treatment. You're correct, gave me a headache trying to read the science behind T therapy. Is this something to consider as an alternative to SRT for recurrence?

No, it's not. It's primarily for men who are castrate resistant. They are the ones whose cancer cells have become hyper-sensitive to androgens and are thus susceptible to being killed by giving them too much testosterone.

SRT is a second attempt at a cure (or a durable remission for us high-Gleason guys whose doctors never use the word "cure") and curative treatments come first. Bipolar Androgen Therapy may be a way to keep castrate-resistant metastatic guys cranking along, conceivably for a long time.

Prior to your answer Peter, I had inquired about TRT for recurrence. The answer they gave was vague as expected. I'm not to the SRT level yet, however I would have a consultation before pulling that irreversible trigger. Below is a cut and paste of my Q & A:

My Message:
Just a quick question, are you using high dose T therapy for Gleason 6 RP guys who have experienced a recurrence and facing SRT?

Answer:
In regards to your question, we have treated patients with TRT even with a Gleason of 10. I recommend scheduling a consultation with us to know your options and learn more about our treatments.
