I am normally one to recommend holding back and researching one's options after the initial diagnosis. But, at age 48, with bilateral PCa in four cores, my humble opinion is that you need to deal with this in a more, rather than less, radical manner.
The recent results that I saw with FLA were good. 13 out of 15 were cancer-free after a few months. But, the cases of bilateral involvement, which you have, resulted in ED, and some head-scratching.
I have read thousands of accounts from patients on this and similar forums. The general consensus is that radical prostatectomy (robotic or open) is the most feasible for someone your age, as you have the best odds to overcome the side effects.
