Natural variation of pre-treatment PSA is about ±20%, just one of the many reasons why PSA kinetics are not used to judge risk. I hope they eliminated other confounding sources of PSA before recommending a biopsy, or perhaps you had other risk factors (e.g., family history, +DRE, African-American, small prostate).
For the biopsy, I like the periprostatic nerve block the best, but one HW member had propofol - nice if you can get it! Blood in semen is common for a few weeks, maybe a little rectal bleeding and in urine for a little while. Tell your doctor immediately if you have fever afterwards.
