It looks like it's rising steadily, now, not bouncing around. I would explore options with a highly qualified doctor.
If I were him I would try to find a medical oncologist at an NCI (National Cancer Institute) Cancer Center who specializes in prostate cancer. Most NCI-designated cancer centers accept Medicare, a quick Google search shows, if that helps.
For example, I have already decided that if my PSA rises, I will head for a nearby university-affiliated NCI-designated cancer center, where I have identified two oncologists who are on the cutting edge of treating advanced prostate cancer.
Others might disagree on the NCI part of this (especially those outside the US!) but I think we can all agree that in the case of metastatic disease, an oncologist who specializes in prostate cancer would be the ideal practitioner to seek out.
One last thing I thought I'd mention in regards to immune therapy--a co-worker of mine is doing Provenge this month. It took some time to get everything approved both medically and in terms of insurance. So I would not hold off too long on getting some expert opinions.
