It says it was done at Bostwick Laboratories in Uniondale, NY. and they call it Prostate Histology.
1/13/2017 There was a 24 core biopsy performed. 3 out of 24 cores detect cancer the first one is Right base, length: 1.5 cm (Bisected) Diagnosis, Adenocarcinoma Gleason 4+3 =7 involving 5% of the specimen.
The 2nd core is right TZ apex #1 length 2.0cm (Bisected) Gleason 4+3 =7 Involving 10% of core. And the 3rd and last says right TZ mid #2 length 1.6cm (Bisected) Gleason 4+4 =8 involving 5% of core. Sorry if I spelled it all out but I didn't want to miss anything and this is ALL new and a little confusing to me. I see a radiation oncologist Thursday morning to talk about how best to try and treat and hopefully cure this and yes I did use the word cure because I need to say it to place myself in the right state of mind. It was explained yesterday by my urologist the the radiation basically kills the prostate along with the cancer cells. Well there it is and a very new road for me to go down but reading all that have had to travel down this road before me does give me hope and encouragement.

Well, Jay, welcome officially to our club. First, the really good news: your pathology is by Bostwick Labs, and it doesn't get any better than that. Second, the decent news: you have 2 7's and an 8. Not great, but could be much worse. Cure is certainly in your vocabulary, especially if the little buggers are still inside the gland where they belong.

Keep that appointment with the RO. Line up consultations with all the expert practitioners you can find. Talk to a surgeon too, just for balance. You have (some) time, with that pesky 8 in there, treatment is not a matter of when, but of how.

Hello Jay....OK so that`s part of the story ....didn`t see stage ...or if still in the capsule info..etc.......be sure to get copies of EVERYTHING ....you NEED it at home to refer too ....and when the bills start coming in you can have it all in a folder ..... I`m getting more and more bills from who knows who ?? just saving them for now. My Year deductable is surely paid . Getting back to your numbers , ,,sounds like you will want to act sooner rather than later ,and like Halbert said ...good chance for a cure !.....stay in touch........David

What about perineural invasion (PNI); did the report say anything about that?
Did Bostwick provide a picture of where the (positive) samples came from; that may help to see if any of them were close to the margin ('positive margins').

I agree with halbert that the report is pretty good and provides a lot of hope for a successful outcome.

SR Sailor, no pictures of where its at although urologist says all 3 cores on the right side of prostate. Nothing mentioned about PNI either.

I'm sorry about the diagnosis, but I do like your attitude!

That's called "high risk," which is often curable. Radiation doesn't really "kill" the prostate, but it does leave it fallow. It kills the cancer inside quite well. If possible, I recommend a trip up to Philadelphia to consult with Eric Horwitz at Fox Chase - he does a combination therapy with external beam and high dose rate brachytherapy that has particularly good results (cure rates as high as 90%). I think I already recommended Dritschilo at Georgetown. You can just call their offices to make an appointment.

Are you having any trouble peeing now? The thing about TZ tumors is that they can swell up right after radiation and interfere with peeing for awhile.

High risk, yes, but every chance of getting all. Like you're plan on seeing a RO. I think that gives you a great shot at cure.

So Jay, you have 3 of 24 with some G8. Bummer, not good. I was only G7 with 12 of 12 and PSA 59. I had HDBRT/IMRT/HT and am cancer free and side effect free at six years.

Let's get on with it and determine what your treatment direction will be. This isn't really that difficult. HDRBT and some IMRT and some ADT (maybe 9-18 months) and you have a long life again. Simple, well proven, and few side effects (maybe none like me).

A lot to decide and not to be taken likely. Please let me know how I might help,

Your pathology is similar to mine. I had 9 of 20 cores positive for PCa, 1 Gleason 8, the rest 6's and 7's

Stay positive.

You are going to beat this

Great place to get your readings. TA is correct in "high risk" category. It is hard to really discuss much more without staging, psa readings, and velocity. Your 4+3 =7 's are treated like 8's rather than 3+4=7's. That would make your three samples be considered like three 8's thus the high risk. Many here have been categorized as advanced or high risk and are doing just fine. However, all in those categories must do all that they can to get the best treatments available. Time is of the essence usually with high risk.
