Hi Guys, I have been on AS almost 2 years now. PSA has remained 5.6-6.4 range. first biopsy 9/15 had 1 pos g6 under 7% of core out of 14. 3TMRI 2/16 showed nothing of interest. Skip to now. Dr suggested its time for a new MRI and Biopsy to be safe . mri show 2 pi-rad 5 areas and 1 pi-rad 3. Decide to get the saturation biopsy transperenial . 40 cores just got results some hgpin could find no cancer. Samples have been sent to JH for second opinion. I figured for sure the PI-RAD 5 was going to be a problem. What a wild ride.

Remember, the PI-RAD score from mp-MRI has nothing to do with the aggressiveness or severity of a lesion of prostate cancer...rather, it has only to do with the likelihood that there is something in the prostate that is at least "clinically significant" enough that it could be found. You've already been diagnosed once, so your PI-RAD scores will probably always be high-ish.

The other thing to also keep in mind is that the reading of mp-MRIs and establishment of PI-RADs scores is still an "art" to many. There hasn't been a procedure that I've seen emerge yet similar to the Johns Hopkins or Jon Epstein 2nd opinion of biopsy slides applied to mp-MRI readings...but maybe someday there will be...

The bottom line is your 40-core biopsy with no cancer found...! Excellent!!

Aren't you glad you didn't make a hasty decision to have surgery or some other overly aggressive treatment before putting on the brakes and saying to yourself..."let's just wait and see if treatment is really necessary..."

Excellent - having AS work out is among the best possible news in our community - right up there with spontaneous remission. Thanks for sharing.

Great news! There are conflicting small studies on the relationship of PIRADS 5 to significant cancer.

From what I have read, there is significant cancer (at least large G 3+3) in the majority of cases. BUT, more and more men on these forums are stating negative biopsy results.

So, you need to keep carefully monitoring, but otherwise, relax and only concern yourself with keeping your next appointment.

Hi Guys, another year of AS has passed 3 psa tests 2/17-6.6 6/17 6.4 1/18 7.0 not sure what to make of it all but psa between 5.4-7.4 for 3 years now .
